You can now print the 2013 - 2014 Religious Formation Registration Form, the Student Information & Permission Form for Parish Program, and Health History and Medical Release Form for Parish Programs and Activities. We have combined these forms into one form. This form is required to enroll your child (children) in the St. Joseph Shrine Religious Formation Program. We are also requesting adults attending sessions to register. These forms are also available at the Family Center Office but if you prefer, you can print (or download and print) the forms from this very spot.
Classes for all grades and adults will be on Sunday mornings from 9:30 AM to 10:45 AM. Tuition is $35 for one child, $55 for two children or $65 per family. Please see Diane if the tuition is a hardship. Tuition is free for the families of catechists and catechist aides.
