You can now print the 2009 - 2010 Religious Formation Registration Form, Student Information & Permission Form for Parish Program, and Health History and Medical Release Form for Parish Programs and Activities. Each form is required to enroll your child (children) in the St. Joseph Shrine Religious Formation Program. These forms are also available at the Family Center Office but if you prefer, you can print (or download and print) the forms from this very spot. Click on the named form below and it will appear in a separate screen. Then, just click on your print button and voila! When done printing close out that screen to return to this one. Classes for all grades and adults will be on Sunday mornings from 9:30 AM to 10:45 AM. Tuition is $35 for one child, $55 for two children or $65 per family. Please see Diane if the tuition is a hardship. Tuition is free for the families of catechists and catechist aides. Questions? Call Diane at the Family Center Office. Registration is taking place during the month of August before and after each Mass. Forms with checks should be handed in at that time.
